Community Hospice of Northeast Florida is committed to providing the services, people, facilities and programs that allow us to care for those with life-limiting illnesses, but there's another vital link in our network of support: you, your friends, your family and your neighbors.

Improving the quality of life for everyone in our community involves working together, doing whatever each of us can to help engage people in end-of-life care issues and to lend a helping hand. This can take many forms, from health care organizations partnering to deliver seamless continuity of care, to committed individuals offering financial support and volunteering their time to Community Hospice and our patients, to each of us taking a meal to a neighbor who is caring for a seriously ill loved one, to simply letting someone know how to find information on this web site for completing their advance directives.
