"It's Your Community Hospice" video now available online

JACKSONVILLE, Florida - Life-limiting illnesses or situations can happen to anyone, at any time and in any place. Likewise, end-of-life care provided by hospice organizations is available in a variety of locations, based on the level of care needed for the best quality of life. It's often difficult for patients or their families to know when hospice is the answer.

Community Hospice of Northeast Florida has developed a DVD to help patients and their loved ones understand when the time is right for hospice care. The DVD walks you through the hospice experience, reminding you that it's your Community Hospice.

How your hospice care is handled is entirely up to you, your loved ones, and your hospice team. Hospice care may be provided in one's home, a nursing home or assisted-living facility, where hospice staff members assist family members and loved ones in caregiving tasks. Inpatients may stay at several hospice locations if they need round-the-clock attention or have need for special treatments or therapies.

Community Hospice Inpatient Hospice Care patients have access to an interdisciplinary team of professionals dedicated to their comfort and care, 24 hours a day. The team may include a hospice medical director, certified nurse practitioner, skilled nurse, social service specialist, certified nursing assistant, spiritual counselor and, upon request, volunteer and bereavement experts.

Established in 1979 as the first hospice program in Northeast Florida, Community Hospice of Northeast Florida provides compassionate care to more than 1,000 patients daily in Baker, Clay, Duval, Nassau and St. Johns counties, through the support of more than 700 employees and nearly 1,000 volunteers. The nonprofit organization has served more than 43,000 children and adults-at home, in long-term care facilities, hospitals and through its two inpatient facilities in Jacksonville. Community Hospice Foundation generates philanthropic and community support for unfunded and under-funded programs, including indigent hospice care, programs and services through the Charles M. Neviaser Educational Institute, and Community PedsCare, a pediatric palliative and hospice program. Care is available to all patients who need it, regardless of their ability to pay.
