Get Support: Community Hospice Care
What are the options for where I can live when receiving care?
Our services are provided wherever you call home – your family home, a nursing home, assisted living facility, hospital, or one of our inpatient facilities.
Community Hospice care in the home: For those who choose to stay in their own home, a family member or friend usually serves as the primary caregiver, with regular visits from the Community Hospice staff to support both patient and family. Equipment and supplies needed to manage the patient's life-limiting condition are provided and set up to make the home environment a safe and comfortable place for the patient to live.
Community Hospice care in nursing homes and other long-term care facilities: For individuals living in nursing homes and other long-term care facilities, the Community Hospice team works side-by-side with the facility staff to ensure that pain and other physical symptoms are effectively managed and to provide an extra level of comfort and support for both patients and family members.
Community Hospice care in one of our inpatient facilities: Community Hospice has four inpatient facilities available for those who require treatment for symptoms that cannot be managed in another setting.

- Earl B. Hadlow Center for Caring:
The 50,000-square-foot Earl B. Hadlow Center for Caring is a residential facility located on Sunbeam Road in the Mandarin area of Jacksonville. This 38-bed facility is situated on 12 peaceful acres and offers a home-like setting with screened porches, a chapel, dining room, lake, courtyard, gazebo and gardens. The Hadlow Center operates 24 hours a day, seven days a week with a full-time medical staff.
- George and Margaret Morris Center for Caring: This 17-bed unit is located at Shands Jacksonville Pavilion and is fully staffed with a medical director and clinical and administrative support staff.
The Morris Center provides short-term care to treat acute pain and symptoms associated with some end-of-life conditions. Once a patient's symptoms are under control, they can return to their regular place of residence.
- Dr. Gaston J. Acosta-Rua Center for Caring opened to patients in January 2007. The 27,000-square-foot facility serves west Jacksonville, northern Clay and Baker counties. It includes 16 inpatient beds in private rooms in a unique homelike setting, complete with gardens, screened-in porches and ponds. Visitors will have 24-hour access. Overnight stays by loved ones are encouraged, as are short visits by family pets.
- Anne and Donald McGraw Center for Caring, located on the Mayo Clinic Campus, opened to patients in early December 2007. The 27,000-square-foot facility serves northern St. John's and southern Duval counties, including the Beaches communities. The center includes 16 inpatient beds in private rooms in a unique homelike setting, complete with gardens, screened-in porches and ponds. Visitors will have 24-hour access. Overnight stays by loved ones are encouraged, as well as short visits by family pets.
Community Hospice care in Northeast Florida hospitals: Patients can be admitted into hospice care while in any of our area hospitals. The Community Hospice team provides an added level of support to the care already provided by hospital staff.
