Community Hospice Care

What are the different categories of care that are provided?

Different patients have different requirements and patients' needs can also change over time. We offer the following categories of care to meet each individual's evolving needs:

Routine home hospice care. We bring care to wherever you call home, including long-term care and assisted living facilities, and we work to make every environment of care comfortable for patients and families. Most often, this means providing care in the patient's own home, where a family member or friend handles day-to-day care and our professionals schedule regular visits to make sure that all the medical, social and emotional needs are being met.

Continuous hospice care. If a patient should have a crisis that needs closer medical attention, we can arrange for inpatient care or around-the-clock skilled nursing care on a short-term basis in your home or wherever you are receiving care. When the crisis has passed, the patient returns to routine care.

Inpatient hospice care. If symptom management cannot be addressed in the home setting, inpatient care in one of Community Hospice's facilities is provided. Then, when symptoms are manageable, the patient can return home.

Respite care. Respite is a period of rest or relief, and respite care is designed to give caregivers just that. When caregivers need a rest from their caregiving responsibilities, patients can stay for up to five days and nights in one of the long-term care facilities with which Community Hospice partners.

Your Community Hospice physician will determine which of these levels of care you need. And while you may never need all four types of care, you can have peace of mind that they are always there for you and your loved ones.
