When a cure is no longer possible, and the burden of care is too great for patients and loved ones, Community Hospice is the choice that offers the greatest opportunity for patients to receive the skilled attention that's right for the end of life. Physicians and other health care professionals are indispensable members of the Community Hospice team. We work with the patient's personal physician to increase patients' access to expert pain management and symptom control, the necessary medical supplies and equipment, home health aides and more. We respect physicians' professional responsibilities, and offer our services at whatever level physicians may need, from telephone consultations to managing a patient's care. Coordinating Community Hospice care. Our services can be provided to patients in a number of settings, including long-term care facilities where the full benefits of palliative care may not be available. In hospitals, Community Hospice liaison nurses oversee the transition from hospital care to hospice care. They relieve physicians and hospital staff of the tasks associated with coordinating such care.
