Job Description: Under the direction of the Psychosocial Program Manager or the Psychosocial Specialist Supervisor, as assigned, the Psychosocial Specialist acts as part of the interdisciplinary care team and manages social service needs of a caseload of patients and families. Coordinates all social service interventions and referrals, including needs assessment, supportive counseling, networking, resource procurement. Contributes to the smooth functioning of services, attends the Interdisciplinary Care Team (ICT) and advocates for patient/family needs with the team and community at large.

Job Requirements: Master's Degree in Social Work from an accredited school of Social Work and three (3) years direct service experience, experience in supportive counseling in group and individual settings, and/or direct service with clients and their families in health care setting, experience in provision of case management services, current state of Florida driver's license with driving record acceptable to agency insurance carrier; and reliable transportation.

Job Description: Under the direction of the Director of Spiritual Care, provide coordination of spiritual services for patients, families and staff; act as liaison between patients/families and clergy/faith communities; spiritual counselor for patients and/or family memebers when request.

Job Requirements: Master's Degree from an accredited seminary or school of theology, minium five (5) years experience in pastoral care, at least one (1) year of which is in a health care setting, one unit of Clinical Pastoral Education (CPE) required, with 4 units preferred, current state of Florida driver's license with driving record acceptable to agency insurance carrier, and reliable transportation, knowledge of policies and practice of pastoral care, various religious doctrines and practices.
